KS diagnosis methods
Jul 3, 2006

Dear doctor, I am 29yo and I write from a town near Napoli. I have started HAART with Truvada and Sustiva since 4 weeks with 252cd4/ml (16%) and VL=939.000. I have lost 6 kg (from 77 to 71) but I eat even more than before. Next week I'll know my blood trends.

Before starting HAART, a pink patch on my hard palate pushed me to have a visit to the oncologist's, who has biopsed it with this result: <> **Can this description exclude Kaposi or can it represent a beginning Kaposi?

In the days after the biopsy some red spot-like patches started spreading on my soft palate, and after 2 weeks of HAART they disappeared, together with the pink papula on the hard palate . **Can this regression be considered a sign of the effect of HAART ("ex-juvantibus") over a KS lesion?

Moreover, before HAART, I had 3 "dermatofibromas" with round shape and red/brown (rust) color (2 on my right and one on my left arm) and telangectasises on the trunk since 8-10 months. **Can teleangectasises be related to the hypothesis of Kaposi and represent an internal organs involvement?

Now, after 1 month of HAART, while the mounth is ok, the dermatofibromas have became 6 + 3 red ones on the right foot and two dots-shaped black lesions on the plant of my left foot. **Can KS skin lesions have a round shape and appear during HAART? **Or can them be benign dermatofibroma related somehow to HIV and/or HAART (as they are starting appearing lately)? **Do you suggest me to have some more biopsies or immunohistochemical analyses or blood analyses? In other words, what do you suggest me to do to exclude or diagnose a Kaposi in early stage? Which organ endoscopy or x-ray or TC or MR... can you suggest?

[[Sorry for my thirst of informations!!! I am afraid that in the town where I am controlled (and in the italian public healthcare management there isn't a early-diagnosis culture, and I think there's something more I can do fast and early. Thank you for your patience and for your precious help to our lives, you medics are angels.]]

That pathology report is not definitive for KS. It is difficult to say with any certainty what you skin lesions represent and I suggest you get them biopsied to be certain and confirm any diagnoses.
