Opportunistic infection thresholds

In the past, I have seen charts that show CD4 counts along side the most common oi's. Could you let me know at which points is an HIV+ person most vulnerable to which oi's or guide me to a chart in a book or on the web?

This information should be readily available in any number of sources: general medical textbooks, like Harrison's or Cecil & Loeb; ID textbooks like Mandell or Bartlett; HIV textbooks like Textbook of AIDS Medicine or AIDS Therapy; and John Bartlett's Medical Management of HIV Infection (known as the Hopkins Handbook). Many websites (including The Body) should have this as well, although I'm better informed about print resources myself than I am about the Internet.
