hyperbaric oxygen and coccidiomycosis

do you have any information on whether or not hyperbaric o2 has any place in the therapy of meningeal cocci? the 32 year old woman patient is doing porrly and her family has asked me about this

I know of no data that support the utility of hyperbaric oxygen in the management of cocci meningitis. This is a tough disease to treat under the best of circumstances, and when I was practicing out West the routine approach was to augment systemic amphotericin B with intrathecal amphotericin, administered via basilar cisternal LP or a high lateral approach by a neuroradiologist under fluoroscopy.
