The combination of tenofovir/emtricitabine (Truvada)would be an ideal component of the cART regimen, providing the kidney function is good. These medications work against both HIV and hepatitis B. Tenofovir is first line against hepatitis B. Both medicatations, overall, are very well-tolerated and among first line agents for HIV.

HIV treatment-naive persons should ideally have a genotype performed to evaluate for any acquired resistance that would impact antiretroviral choices. The 3rd antiretroviral agent would depend in part on any other health issues, CD4/HIV RNA and medication drug-drug interactions.
