HCV and condom
Jan 22, 2012

hi doc, recently in wikipedia i saw CDC has not recommended use of condom for serodiscordant (for HCV) monogamous heterosexual couple. Is it not risky to do so? Can HCV and HIV be transmitted through unprotected receptive and insertive fellatio if there is no blood in saliva? regards.

Response from Dr. McGovern

The CDC basically states that the risk of HCV transmission among steady sex partners is low. But it does not state that there is NO risk. I counsel couples that the risk is very low (about 1 to 2 percent) and that the decision to use condoms or not is up to the couple. this is a very personal decision. Transmission through saliva has not been documented.

The risk of HCV transmission through sex increases among persons who have multiple sex partners, who have rough sex which causes trauma to mucosal surfaces, and among men who have sex with men. Concomitant infection with HIV seems to be associated with an increased risk of transmitting HCV too.
