|Fibrosing Cholestatic Hep C Pre-transplant
Oct 4, 2008
My husband - HIV, HepC, Hemophilia. Failed peg & ribavirin x2. Recently had acute increase in liver enzymes and Bili. All were normal 9/3 and 12-14 x normal 9/15. Had bx to reveal cirrhosis and Fibrosing Cholestatic Hep C. We have started transplant workup and go to Cedars Sinai on Monday(transplants in co-infected). His doctors here have restarted interferon besically because there is no other treatment available. But they don't have experience in pre-transplant FCH. Not many do. His last interferon treatment was 2/08-8/08 and was stopped for failure to respond. He had many side effects - debilitating joint pain, hyperammonemia, hyperuricemia, immunosupression (t-cells 580 in march and 70-90's currrently with undetectable HIV to date!) We are ok with the transplant option and are eager to have a new life with a new liver. But we still want to get as much under control now so we don't have to deal with it post-transplant. Any help would be greatly appreciated!

This is an extremely complex situation and it is difficult for me to comment without knowing more details. I would want to be certain that your transplant physicians also agree with the plan on retreatment by your current physicians since a smooth transition is important.
My best wishes to your husband for a successful transplant.
