HIV and Hep-c
Nov 12, 2007

I've been trying to decide if I should start hep-c treatment. I've been told that genotype 1 is the hardest to successfully treat in combination with HIV. Any thoughts on this subject?

Response from Dr. McGovern

Plenty of thoughts: 1. You should weigh if this is a good time for you to start therapy in light of your job, family, etc.. 2. You should review your liver biopsy with your physician to determine if you need treatment right now. If minimal disease, you could wait. 3. If you do go on treatment, I would suggest weightbased ribavirin with pegylated interferon alfa 2a (Pegasys) which was approved by the FDA for patients with HIV/HCV. 4. I would also recommend viral load testing at 4 and 12 weeks to determine your viral load response. If it is favorable, you should do well regardless of your HIV seropositive status. 5. Also review your HAART meds to be certain there are no drug interactions with ribavirin.

Best of luck.
