what are the best hiv meds to be on if you're going on rebetron for hep c?

We don't know the best to be on. We do know two to avoid, however - as both AZT/retrovir and D4T/zerit can compete in the cell with the ribavirin part of the rebetron - so you would need to use meds other than those two.
Other than avoiding those two, there is debate but only a little info. In general you would need to avoid meds that often can lead to hepatitis - but if you are already on some meds that can occasionally cause hepatitis and are not having any problems - then you probably don't have to switch. Keep in mind, however, that virtually all antivirals can sometimes have some problem in the liver (tho some much more than others) - and those with Hep C appear to be more at risk for having such trouble.
Another problem with those who have Hep C is that it is hard to even know how to judge liver trouble - since there can be increases in liver enzymes and we are not always sure what significance these bumps will have.
