Dear Dr. Dietrich¡ i am a hiv/hcv co infected 41 years old female. i have been on pegasys once a weed and riv 1000 mg a day, hematological toxicities have been under control and my hiv is also well controled. After 12 month of treatment hcv vl has been up and down but was never negative.i am genotype 4 and have cirrhosis. I would like your opinion on treatment strategies. Best regs God Bless you Dr. ¡
Response from Dr. Dieterich
You have several options. You may want to stop for 3- 6 months give yourself a rest and then start again with Peg and RBV maybe at a higher dose for the first 3 months. Alternatively, you can go on maintenance peg at half dose without the RBV. Either way you should probably also get on a transplant list where they do HIV+ liver transplants, just in case. Genotype 4 should have a better prognosis than Geno 1. Good luck! DTD
