If you have extra fat related to lipohypertrophy, then this may be a complication related to cumulative exposure to HIV medications, and protease inhibitors are the leading culprit. There are some studies that have shown improvement in lipohypertrophy when protease inhibitors (Kaletra in your case) are switched for non-nucleoside reverse transcriptase inhibitors (NNRTIs). If you have been on treatment with this class in the past and have virus that is resistant to these agents, then switching to them (Viramune or Sustiva) is not an option. However, we now have a second generation non-nucleoside reverse transcriptase inhibitor called Intelence, so this may be an option. Additionally, an integrase inhibitor called Isentress is now available and I have seen improvement in lipohypertrophy after switching protease inhibitors to this agent.
