What should I do about my 50 KS lesions?

Dr.Bruce,I have approx.50 KS lesions on skin.I have been on HAART meds. for around 6 months with great results. Viracept is one of the drugs in the cocktail,suposed to be good for helping with KS also. I want to know besides panretin gel,is there any other way to (get rid of) KS besides chemo,which my doctor say is not advisable for me at this time?? I need another opinion. What's your take?

If you have 50 KS lesions, you are certainly a candidate for chemotherapy. I'm not sure why your doctor says it's not advisable at this time. The chemotherapy, which we give for AIDS-related KS, is Doxil or DaunoXome. These are chemotherapy agents formulated in a special manner called liposomes. The liposomal formulation allows these drugs to be given with very few side effects. I know of very few patients who can not withstand chemotherapy. I've successfully given chemotherapy to patients with very low CD4 counts and in the midst of opportunistic infections.
Panretin is a gel, approved for the treatment of KS, which you can apply directly to the lesions. Fifty lesions may be a bit too many to treat with panretin gel. Have you been seen by a cancer doctor yet? If so, I'd be interested what s/he says.
